In a medium saucepan, heat chocolate over medium heat, whisking occasionally. Whisk in cream until smooth; gradually whisk in milk until smooth. Stir in vanilla, salt, and nutmeg. Let simmer for 10 minutes. Garnish with marshmallows and caramel, if desired.
